Regional core hospital with manpower shortage

【Abstract】
After childbirth, there are many mothers suffering from solitary child-rearing,
and it is said that conditions are set for easily becoming mentally unstable
due to the burden of child-rearing immediately after giving birth. However,
the current situation is that no “postpartum care” support after discharge
has been established due to limited human resources and time.
We conducted actual health examinations for new mothers in order to elucidate the current condition, effectiveness, and problems surrounding the second week health examination. It was shown that, even in environments where family support is available, child-rearing anxiety is high, and postpartum care is necessary to reduce anxiety levels. Midwives in the wards performing health examinations were effective in alleviating anxiety in the mother after discharge. Concrete measures are required for implementation and working towards an enriching postpartum care program was seen as the greatest challenge.

【Abstrct】
An average of 200 patients per day visit the internal medicine outpatient clinic, the average waiting time is 4-5 hours according to a questionnaire. In order to elucidate how patients felt about long waiting times, a questionnaire was given to outpatients for a nursing study in 2016. A waiting time project team was formed in 2016, and we report the reform measures that were proposed based on the answers to the questionnaire.

Survey of attitudes toward nursing staff who conducted training sessions
for practical guidance |

【Abstract】
In hospital A, seven years have passed since receiving of practical guidance
was started. The team of instructors is working toward the goal of nurturing
students in full force of the hospital. From the results of the questionnaire
survey every year, it was found that the number of nursing staff, who thank
that they were not good at practical guidance, was increasing. On the other
hand, it was found that 60% of nursing staffs were interested in the workshop
for practical guidance, showing us the necessity of holding the workshops.
Therefore, we held the ones, of which the aim was going forward to effective
practical instruction by decreasing awareness of performing practical guidance.
As a result, although holding the workshops was not so effective for decreasing
the awareness of nursing staff concerning practical guidance, it became
clear that it was effective for changing behavior level such as self-development
and stimulating the interest of individual nursing staff on students. Hereby
we report the detail of the survey.

Effect of verbal communication
in cardiac catheterization laboratory

【Abstract】
At present, we do not carry out visiting patients before examination employing
cardiac catheterization. Of course, just before starting it we explain
briefly how to express physical and mental pain of theirs and during it
we ask and check their conditions. However, there have been some patients
who move suddenly without saying anything, sometimes leading to unexpected
high risk. Therefore, we investigated the effect of verbal communication
such as asking some questions by nurse to the safety of examination by
using questionnaire survey. As a result, it was suggested that verbal communication
during examination efficiently relieved tension of the patients and made
them easy to express their own thoughts.

Efforts concerning warmth retention during large intestine endoscopic submucosa dissection (colon ESD)

【Abstract】
At A hospital patients wear gown-type underwear for upper body and short pants of knee-length with a hole for lower body during endoscopic submucosa dissection (ESD) of colon. This means that patient must undergo operation with insufficient coverage by underwear for lower body. The average operation time is 82 minutes, which suggests that it is difficult to keep warm lower body when a large part of the skin is exposed to air and the posture is forced to be exchanged several times. Therefore, we thought that patient could undergo ESD without feeling cool or cold if they could wear something for less exposure to air. In the present study, we examined whether adding leg encloser could keep warm the patients during operation. As a result, the temperature in popliteal fossa did not change regardless of the usage of leg encloser, however, that in lower thigh and dorsum pedis was significantly higher by using leg encloser. Subjective feeling of warmth in the examined patients was confirmed, too. We think that it is important to continue the attempts of this kind to keep warm patients during ESD.

【Abstract】
It has become the era when one in 11 eleven Japanese women suffer from
breast cancer. People suffering from breast cancer begin to increase from
the late 30s, peak in the late 40s to 50s of theirs, and are the leading
cause of death among 30 to 64 years old. Early detection and early treatment
of breast cancer are important, so breast cancer screening and self-palpation
are regarded effective means. In Mutsu City with a low rate of visiting
breast cancer examination, we thought that focused efforts were necessary
to increase the number of breast cancer examinees, and as a result of the
project team's enlightenment activities, the rate apparently increased.

Perspectives on Videoendoscopy and Videofluoroscopy for Swallowing Examination
in My Department |

【Abstract】
Videoendoscopy (VE) and videofluoroscopy (VF) are classic examinations
for examining swallowing function. However, VF is considered the gold standard
at my department due to the quality and quality of the findings. In VF,
I am cautious of laryngeal penetration and aspiration which are associated
with delayed swallowing reflex and residue in the pyriform sinuses. In
VE, I pay careful attention to pharyngeal saliva pooling, movement of vocal
cords, and malignancy. Especially in VE which requires surface anesthesia
of nasal cavity as the pretreatment, saliva pooling in the pyriform sinuses
is a simple and objective assessment item; therefore, I recommend its application
in screening for dysphasia.

【Abstract】
At the meeting within the department of nutrition management division, there was an opinion that no special feeling was felt for the celebration meal after birth, and a questionnaire survey was conducted for 62 inpatients who ate the celebration meal between Sep 15th and Nov 17th 2017. As a result, 30 answers were obtained and 90% of the responses were satisfying ones. Although there was no dissatisfaction, there was an answer that "I want to eat a special meal different from regular one". Therefore, we reviewed the menu and dishes. It was the issue to provide a ceremonial menu as a standard menu. Because there were a lot of responses “I wanted to eat the beef steak”, we have repeatedly attempted to cook the beef steak for the purpose. However, we could have not solved the problem to date. We hear the opinion that the meat is hard. We would like to continue to cooperate with cookers concerning the issue.

【Abstract】
The radiology outpatient nurse is involved radiological diagnosis and radiotherapy, and correct understanding of radiation is necessary along with deep knowledge of various examinations and protective methods. In addition, a collaborative approach to medicine cannot be separated from diagnosis or therapy. In order to be involved in patient-centered medicine with doctors and radiological technologists, and to provide the best care for the patient, I attended a training course and report on the learnings from this course.
