Radiosensitivity estimation of human brain tumors using primary culture
technique and the cytokinesis-block micronucleus assay |

【Abstract】
Clinical relevance of ex vivo radiosensitivity estimation,
using primary culture technique and cytokinesis-block micronucleus (CBMN)
assay, was investigated in 7 patients with primary brain tumors. The tumors
consisted of 4 astrocytomas, 1 oligodendroglioma, 1 glioblastoma multiforme and
1 teratoma. They underwent partial resection of the tumor and 6 of the 7 patients
received postoperative radiotherapy. CBMN assay was performed using primary
cultured tumor cells that originated from the resected tumor specimens, and the
parameters, the average of number of micromuclei per binucleated cells (MN/BNC)
induced by 2 Gy irradiation after subtraction of that for control (MN2) and the
MN/BNC increasing rate induced by irradiation (MNIR) were obtained. In the 6
patients who received postoperative radiotherapy, tumor response was evaluated
by serial MRI or CT scan. Although MN2 and MNIR showed some differences among various histological
types, those for oligodendroglioma and glioblastoma multiforme were suggestive
of being radiosensitive and radioresistant, respectively, when considering
those for fibroblast from human normal tissues. Furthermore, it seemed that the
values of MN2 and MNIR of the tumors were related to the tumor responses by
subsequent postoperative radiotherapy. These results suggest that our method
could be useful for predicting clinical outcome of treatment to human brain
tumors. |

【Abstract】
The Center of Radiology/Radiotherapy newly introduced latest radiation
therapy planning system “RayStation®”in 2016. In 20 cases that were planned
radiation therapy using this system, we performed dose verification using
three devices (dosimeter, cylindrical detector, film) from January to June,
2017. We obtained good results for all cases, so reported them.

A case of Biliary Metallic Stent Migration into the Duodenum |

【Abstract】
A 77-year-old man with obstructive jaundice due to cholangiocarcinoma underwent
placement of a covered self-expandable metallic stent (SEMS) (10×80 mm)
inside the common bile duct at another hospital. Four months later, he
presented to the author’s hospital with acute cholangitis. Computed tomography
revealed that the covered SEMS had migrated into the second portion of
the duodenum and several stones were present upstream of the narrowed bile
duct. The migrated covered SEMS was removed without complications by endoscopy
and a new uncovered SEMS was inserted to prevent migration. Furthermore,
endoscopic mechanical lithotripsy was performed for the removal of bile
duct stones. Endoscopic stenting is an effective and palliative treatment
in patients with malignant biliary obstruction. SEMS improved stent patency
and covered SEMS overcame tumor ingrowth. However, careful attention must
be paid to stent migration, a rare complication that involves covered SEMS.

Nursing of a family with end-of-life patient
- Support for independence after discharge - |

【Abstract】
In recent years, the family structure has changed to various forms, but
when one of the family members is at the end-of-life, changes in family
function, roles, lifestyles are produced, and necessitates the expansion
of the emotional support and coping action. While this example was a patient
who was hospitalized herself, it was predicted that her role would change
and self-function decline as her husband was suddenly hospitalized and
neared end-of-life. Therefore, using CNSFaceII, care was provided according
to her needs. At the same time, a support system was created for the patient
after discharge by sharing information between the family and various professions.
We think that it is important to intentionally create a relationship with
the family and create a care system by cooperating with other facilities
and professions.

【Abstract】
Judgment of the effectiveness of radiotherapy against brain tumors is often done morphologically using contrast MRI. Recently, we have attempted to add diffusion tensor images and diffusion tensor tractography when performing MRI examination and add functional assessment to the evaluation. In this study, we examined the effect of treatment on 3 cases of metastatic brain tumor, and as a result, it became possible to judge the therapeutic effect by visualization of active nerve fiber bundle as an indicator of function, which could not be analyzed previously by conventional MRI examination. |

【Abstract】
In recent years, the dramatic evolution in the field of information technology
has enabled electronic storage within electronic medical records, not only
image information but also data of physiological examinations. Although
our hospital, too, introduced a system two years ago, it is still difficult
to manage the physiological test results. We frequently receive one question,
“why can’t we see electrocardiogram on the system of electronic medical
record in our hospital?” In response to the inquiry, I would like to state
the necessity of introducing electrocardiogram system to the system of
electronic medical record with the background. |

【Abstract】
We analyzed the Semmes-Weinstein monofilament tests (SW test) that were
requested to the Occupational Therapy (OT) Office, Department of Rehabilitation
in the past 5 years at our hospital. The SW test is a sensory assessment
tool consisting of a set of 6 or 20 monofilaments to identify deficits
in threshold perception and pressure. In peripheral neuropathy such as
entrapment syndrome, threshold is assessed and mapped, describing the level
and degree of nerve damage. At our hospital, the number of SW test prescribed
is near the top of assessment tests prescribed to outpatients throughout
the year. The most common diagnoses are carpal tunnel syndrome and cubital
tunnel syndrome. Here we introduce the actual data results of 237 patients
of the past 5 years and describe the actual mapping from preoperative and
postoperative tests using a case study. |

【Abstract】
According to the revision of medical fees in FY2016, dietary medical expenses
(I) and subjects of nutritional diet guidance were expanded, and the contents
of instruction was expanded. This revision is thought to become a tail
wind for managerial dieticians and dieticians who are working as nutrition
/ diet experts. The future problem is that necessary nutrition management
for patients can be performed continuously from the time of hospitalization. |

【Abstract】
At our hospital, the Department of Pharmacy has been tasked with switching
to generic drugs and expanding the contents of confirmation for prescriptions
brought by patients with the introduction of DPC from April, 2016. As a
general rule, by October 2016, outpatient prescriptions had changed to
external prescriptions and actively worked on improvement of new tasks.
Here, we report on the results and assessment of the work restructuring
of FY2016. |

【Abstract】
We investigated the performance status of duties in the center of Radiology/Radiotherapy in Mutsu General Hospital according to the radio diagnostic /-therapeutic modalities. Based on the current results, we are trying to improve the performance and efficiency to contribute to the benefit and profit of both our center and the hospital. |

【Abstract】
We reported on the business statistics of the Department of Clinical Engineering
in the 2016 fiscal year. At our department, we mainly perform equipment
management, cardiovascular-related work, and blood purification. In equipment
management, inspection of pumps etc. had shifted without change. Inspection
requests for biometric information monitors were increased. The cardiovascular
service had shifted without change. With regard to blood purification,
treatment requests other than hemodialysis were increased. |
