Usefulness of semi-solid medical food re-administered after percutaneous
endoscopic gastro stomy |

【Abstract】
Background:Even though there is no problem during preoperative enteral
nutrition, some patients receiving enteral nutrition after percutaneous
endoscopic gastrostomy (PEG) suffer from digestive symptoms, including
vomiting and diarrhea. The aim of the present study is to investigate the
usefulness of semi solid medical food, which is more physiologic than the
other nutrients.
Methods:My retrospective study included 91 patients who I performed PEG
as the attending physician from August 1, 2011 to March 31, 2019. There
were no instances of digestive symptoms during enteral nutrition before
PEG. The types of nutrients re administered after PEG were divided into
three categories: semi solid medical food (n = 20), polymeric form ula
(n = 26), and elemental diet (n =45). I compared the rates of digestive
symptoms within one week after nutrient re administration.
Results:The digestive symptom rate of semi solid medical food group was
significantly lower than that of polymeric formu la group [0% vs. 26.9%
(7/26), p <0.05] and was similar to that of elemental diet group [0%
vs. 2.2% (1/45), p = 1].
Conclusion:The present study revealed the usefulness of semi solid medical
food as well as elemental diet re administered after PEG for th e patients
who had no digestive symptoms during preoperative enteral nutrition.

【Abstrct】
In the Mutsu General Hospital,We carried out 50-60 examination with emergency
cardiac catheterization every year, but as a result of Acute Coronary Syndrome
developing in 2018 whether there was a difference for a myocardial injury
by a difference of the distance to our hospital after an acute coronary
syndrome developed, and having compared it using the numerical value of
Creatine Kinase, Creatine Kinase -MB type, troponin T of the myocardial
marker targeting at t he patients whom I treated by emergency cardiac catheterization
inspection, the distance to our hospital showed a high price to the myocardial
marker so as far off, and it was thought that there was the myocardial
injury which was stronger than the patient that distance was near.

【Abstract】
We examined the outcomes of curative anti-cancer therapy for esophageal
cancer and non-small cell lung cancer, employing 3 types of survival rate
based on the survey data of hospital cancer registration. As a result,
it was suggested that they were within the level of national standard,
although the patient number and the observation period were not sufficient.
We would like to further investigate the treatment outcomes for malignancies
in the other region.

Simulation of Critical Emergency Cesarean Sections at Night and on Holidays

【Abstract】
Since 2015, in order to establish a system for critical emergency cesarean
sections at night and on holidays , Hospital A has been preparin g manuals
and conducting simulations in line with the hospital`s present situation.
Hospital A verified that the continuous critical emergency cesarean section
simulatio s were effective in creating a system. As a result, the anesthesia
ward staff established an anesthesia initiation system which made it possible
to shorten the time from the start of surgery to delivery of the baby.
The critical emergency cesarean section simulations at Hospital A led to
an increase in awareness of multidisciplinary cooperation. By looking back
at the results of these simulations, issues were clarified and a new system
could be built.

【Abstract】
Hospital A has followed the Periodic Endoscope Examination Protocol (hereinafter
referred to as the protocol) as outlined by the Japan Gastrointestinal
Endoscopy Engineers Endoscope Safety Management Committee since FY2017.
Regular culture inspections on parts of gastrointestinal flexible endoscopes
(used to review the first and second parts of the duodenum) such as the
outer surface, suction line, air line, water line, special line, forceps
button, as well as inspections of the storage location of the endoscopes,
are conducted once a year. As a result, the routine endoscopy culture test
is believed to be effective as a means of confirming that endoscopes are
reliably cleaned, disinfected, and stored, and should be continued.

【Abstrct】
Special District Authority of National Health Insurance Shimokita Medical
Center “Oma Hospital,” located in Oma Town in the northernmost part of
Honshu, is the medical agency which supplies medical treatment to an area
including Oma as well as the neighboring villages of Kazamaura and Sai.
Furthermore, the distance to Special District Authority of National Health
Insurance Shimokita Medical Center “Mutsu General Hospital,” a nearby impatient
medical institution, exceeds 40 kilometers, so Oma Hospital is often required
to provide emergency medical care. In May of 2018, a request was made to
implement a system for transporting blood products from Mutsu General Hospital
to Oma Hospital in cases where blood products were more urgently needed
in Oma. The effects of the creation of the supply system for blood products,
as well as the reduction of blood disposal, are reported here.

Introducing Exercise Therapy for Dialysis Patients at the Office for the
Department of Rehabilitation

【Abstract】
Dialysis patients tend to have reduced muscle strength, tolerance for exercise,
activity, and quality of life due to long periods of bedrest during hemodialysis.
In recent years, there have been many reports that exercise therapy during
dialysis can improve a patient’s physical capabilities. At the office for
the department of rehabilitation, outpatients were introduced to exercise
therapy during the course of dialysis. Their physical capabilities at the
beginning of the exercise period were compared after three months and six
months. A questionnaire of their exercise therapy experiences was also
conducted. Results showed that the physical capabilities of patients improved
and were maintained. From the questionnaire results, overall opinions of
exercise therapy were positive, and all target patients expressed the desire
to continue exercising. In the future, we would like to make periodic evaluations
and encourage outpatients to continue exercising in the hopes of improving
their quality of life.

【Abstract】
Mutsu General Hospital implemented the DPC system in April 2016. Since
then, the pharmacy department has contributed to hospital profits by promoting
the switch to generics. However, since April 2018, the method of calculating
generic drug use in the DPC system addition has changed, and it has become
necessary to further promote switching to generic drugs. This report describes
the results of these activities and their evaluations in fiscal year 2018.

【Abstract】
The number of hospital meals served (I) in 2018 was slightly higher than
the previous year. Due to an increase of hospitalized patients with chewing
and/or swallowing problems, there was an increase in the proportion of
non-additional special meals. Examination of dietary forms and lack of
manpower for cooking have been problematic. Although dietary instructions
have remained unchanged, these challenges create obstacles for future progression.

【Abstrct】
This document presents various statistics of the clinical engineering department
during the 2018 fiscal year. The duties of the clinical engineering department
include equipment management, cardiovascular work, and blood purifications.
Regarding equipment management, requests for inspection of foot pumps and
biological information monitors are increasing. Regarding cardiovascular
work, catheter-related cases are increasing. Regarding blood purifications,
the number of dialysis treatments has increased with the increase of dialysis
machines; however, requests for treatments other than those of hemodialysis
have decreased.

【Abstract】
The Mutsu General Hospital Nursing Bureau has implemented "Target
Management" as a theme since 2007. It has been difficult to secure
enough time for interviews to confirm goals, but it is necessary to make
each staff member aware of their roles. Regarding the first theme of 2018,
“strengthening of teamwork”, results were almost as expected. Regarding
the second theme, "strengthening post-hospital discharge support",
results were below the target. Regarding the third theme, "responding
with compassion”, results were as expected. In the future, we would like
to continue following the target management plan for nurturing human resources,
improving the quality of nursing, and participating in hospital management.
